Imagine a hacker who's always two steps ahead, he doesn't make mistakes, he doesn't get caught, but how? and why?

What I want to discuss today is something I think is very true in various aspects in this world, but especially in the world of cyber-warfare. We've seen numerous hackers penetrate government systems, SCADA/ICS etc. 90-95% of which have been caught. So what about the remaining 5? Well, two hackers come to mind when I think about the very few people who's been able to evade LE.
OrginalGuy & (cant remember his name)
These two (to the best of my knowledge) have been able to escape prosecutions from LE. You might only be familiar with OriginalGuy, however X has achieved significantly bigger achievements. He singelhandedly took out a hacking group & sent vast amounts of BitCoins to an anti-ISIS organisation.
He at this point is known as the Ghost on the internet because he from time to time pops up on the surface when he wants to expose his hacks, other than that, you can find him on his twiter profile which obviosuly is inactive for most of the time.
What I want to say here, is we all strive to get to this level of superiority where we're untouchable, because who wouldnt that type of power. But aside from the technical proficiencies, what else does it require to become so good that nobody can catch you?
I mean, let's say you have a Linux OS with VPN's, Tails, you got 5-6 hops and are piggybacking from IP to IP. Even with all of this and absolute discretion, what about the human aspect of it?
We all make mistakes, and one day you might find yourself with only 2 hops instead of 5-6.. That could be because of network failure, software failure or human failure meaning you forgot to make sure you had everything up and running before you started your business. That is why I want to discuss this, we all make mistakes regardless of our level, and it;s because of that I believe there are various concepts that have to be implemented into our lifestyle in order to achieve this 'level' or get close to it.
Secrecy is Security & Security is Victory. In other words, keep your mouth shut about your business and who will be able to catch you?
Stop while you're ahead, the good ol' saying which many of you likely know.
Operate in the Shadows & Get Out. Another one which many choose to neglect because they're too greedy for attention. Not having that twitter profile will increase your chances so much.
I could go on, but like I said, Secrecy is Security & Security is Victory ; )
